Following promulgation of the Nuremberg code in 1947, the ethics of research on human subjects has been a challenging and often contentious topic of debate. Escalation in the use of research participants in low-income countries over recent decades , has intensified the debate on the ethics of international research and led to increasing attention both to exploitation of vulnerable subjects and to considerations of how the 10:90 gap in health and medical research could be narrowed. In 2000, prompted by the (...) discussions over several years that led to the US NIH launching a capacity building programme on research ethics for members of research ethics committees in developing countries, we advanced a ‘new look’ for the ethics of international research.1 Since then progress has been made on several fronts.First, our ideas—considered somewhat radical and impractical at the time—have been provocatively addressed by scholars who have either contested them or advanced similar conceptions of what obligations international researchers have to research participants and communities in low income countries before, during and after clinical trials. Second, those researchers who have been sympathetic to our ideas have either endeavoured to put these into practice or have investigated the feasibility of doing so. Third, the intractability of the 10/90 gap and the escalation of interest in global health have sensitised many to the need to amplify the uptake of these ideas in practice.Here, we briefly review the conceptual and practical developments in international research ethics. Polanyi’s philosophy of “man in thought,” by all appearances, chronologically and structurally, seems to be founded on his epistemology. Polanyi’s epistemology of tacit knowing as integration is teleological. By his “ontological equation,” he patterned comprehensive (and complex) entities as emergence on his epistemology. This forces him to make puzzling formulaic statements which land him in trouble with fellow scientists. The equation also lends itself to unwarranted problematic interpretations. The exploration leads me to suggest that Polanyi may be understood as a (...) “rational realist” who insisted on a tacit knowledge version of interactionist mode of mind-body relation. (shrink)

Dr. Brown, a hospitalist, admits Mark, a patient transferred from a local jail for management of cellulitis. The patient, who was taken into custody two days prior to hospital admission, has a history of intravenous heroin use. Mark explains that he had been prescribed buprenorphine-naloxone maintenance therapy for opioid use disorder for several years prior to being arrested and had not used other opioids during that time. As a policy, the jail where Mark is detained does not prescribe opioid agonists, (...) and his maintenance therapy was stopped upon his arrival there. Dr. Brown discovers that Mark is diaphoretic and appears distressed. Mark's symptoms suggest to Dr. Brown that, in addition to having cellulitis, Mark is actively withdrawing from opioids. Mark tells Dr. Brown that he has felt “horrible” since his buprenorphine-naloxone therapy was stopped and that he now has intense cravings for opioids. He asks Dr. Brown to help alleviate the withdrawal symptoms. Dr. Brown, who is accustomed to treating opioid withdrawal with opioid replacement therapy, wonders if she should initiate ORT for Mark while he is in the hospital. (shrink)

We present a model of steady state solute and water reabsorption along the rat proximal tubule. Major co-and counter-transport systems in the apical and basolateral cell membranes are described using kinetic descriptions based on data from the flows and solute concentrations along the length of the proximal tubule as a function of filtration rate and peritubular solute concentrations. We show that for many aspects of proximal tubule transport physiology this kinetics-based model is an adequate representation of the mammalian proximal tubule.

Some books are like parents, grandparents or old friends. They have been with us from our earliest days and one treats them almost with familiarity. They belong to one's youth and the recognition that they have been around for months and years keeps company with surprise. For philosophers such a book is A. J. Ayer's Language, Truth and Logic, first published over fifty years ago in 1936. There is a sense in which a similar point may be made about some (...) individuals, but discretion and good manners should deter us from succumbing to the philosophical disease of pressing an analogy too far. Suffice it to say that over a period during which most English speaking philosophers were content to work within a context which was significantly influenced by Ayer's clear and cajoling formulation of a twentieth century of empiricism, F. C. Copleston provided one of the few distinctive alternative philosophical perspectives on major metaphysical questions. This essay will reflect upon the influence of Ayer's Language, Truth and Logic some fifty years after its publication, upon the philosophical discussion of religion and theological questions. The main purpose of this book is to offer a comparative evaluation of the early works of Marx and Lukács, "these two most important texts of 'unorthodox' Marxism". Feenberg refers here to the Economic and Philosophical Manuscripts on the one hand, and History and Class Consciousness on the other. At a somewhat more general level, Feenberg writes in defense of, or more accurately in preparation for a new stage in, the philosophy of praxis. To proceed directly to his conclusion, Feenberg's (...) analyses culminate in the recommendation of a synthesis between Marx's concept of the internal relation between subject and object, or their participatory identity, and Lukács' mediation-theory of rationality, according to which reification is an intrinsic element at the dialectical level of reason, which is thus a mediation of nature on the one hand, and the subject-object relation on the other. In less complex terms, Feenberg recommends the development, from elements of Marxist and Lukácsian doctrines, of a "non-creationist concept of practice", i.e., one in which nature is not created by man, a concept that is also not pure objectivism, i.e., that prevents a complete identity of subject and object. Feenberg thus perceives the central problem in the early Marx and Lukács as one that links them more intimately with Absolute Idealism than his own analysis makes explicit. The "fundamental flaw in the philosophies of practice of Marx and Lukács" is, to put it as briefly and clearly as possible, that in these doctrines, man is still a creator-god. (shrink)

In the Neoplatonic schools, introductions to logic, and the Categories in particular, would begin with a list of ten different questions relating to Aristotle's philosophy and his ideal interpreter and student. Olympiodorus' own introduction to logic follows this pattern; he expands on the remarks of his own teacher Ammonius of Alexandria, and closely models his discussion on his predecessor's work. In the standard list of ten questions that must be discussed in an introductory philosophy course, the third relates to the (...) subject with which the student is to start his or her philosophical education.
